Services
This office is the custodian of the institution’s academic and curricular records. It provides the following services: registration and enrolment of learners, maintaining and updating of learner’s academic records, processing and issuance of diplomas, certificates, permanent records, and various types of certifications pertaining to the learner’s academic records.
All students seeking admission for the first time to San Jose Academy are required to submit to the Guidance office the following requirements:
Original and photocopy of the following:
- PSA certified Birth Certificate (2 photocopies)
- Baptismal Certificate (2 photocopies)
- Confirmation Certificate (for incoming Grade 7)
- Report card (Form 138)
- Medical Certificate
2 pieces recent 2×2 ID colored picture
1 piece recent 1×1 ID colored picture of both parents
Immunization Record (for Nursery to Grade 1)
Certificate of Good Moral Character from the school last attended duly signed by the Principal (for Grade 4 to Grade 12)
For Senior High School – Completion Certificate duly signed by the School Principal, Original Copy of the National Career Assessment Examination (NCAE) results.
For New students
1. APPLICATION FOR ADMISSION
-Go to www.snja.edu.ph and click the "Enroll Now" tab. Or
https://snjasis.orangeapps.ph/snja/admission
– Fill out and complete the application form.
For walk-in applicants, proceed to the Guidance Office and complete the online application
form. Bring your admission requirements.
2. INTERVIEW AND SCREENING
-For ECE and JHS applicants, an admission test fee should be settled at the Accounting Office.
Note: Admission test fee is WAIVED for incoming Grade 7 applicants coming from public
schools.
-Proceed to the Guidance Office and submit your ADMISSION REQUIREMENTS. Present the
official receipt to take the admission test and interview.
For Old Students, proceed with Step 3.
3. ENLISTMENT AND ASSESSMENT
– Proceed to the Computer Laboratory to enlist and choose the mode of payment for the
assessment of fees.
– For JHS and SHS proceed to the Registrar's Office for ESC and SHS Voucher Updates and
Application
4. PAYMENT
– Pay the upon enrollment fee via on-site or online payment.
5. ID PHOTO TAKING
-Students must wear proper haircuts and hair color.
– Secure payment and receipt at the Accounting Office.
– Proceed to the MIS Office for the ID Photo-taking. ID cards will be released on the same day.
6. BOOK RELEASE
– Proceed to the Grade School Laboratory and present copy of the booklist and receipt of
payment to the personnel-in-charge.
